Introduction / Background
Organophosphorus(OP)compounds are a diverse group of chemicals used in the domestic,agricultural and industrial settings.Examples include insecticides(malathion,parathion etc,herbicides,and nerve gases(sarin,tabun and VX)developed in the middle of the 20th century.Thier use in chemical warfares and in terrorist attacks is a growing concern now a days especially in the western world.Organophosphates are responsible for a number of human poisoning particularily in developing countries.Primary mode of action of OPs is thier irreversible inhibition of acetylcholinestase(AchE)enzyme that causes accumulation of neurotransmiter acetylcholine at muscarinic and nicotinic synapses and in the central nervous system(CNS)causing a variety of toxic effects.Intentionl and accidental poisoning by OPs is leading cause of premature deaths in developing countries like Pakistan,India and Srilanka.OP poisoning is a common,rapidly progressive and potentially fatal clinical entity.Such patients need careful thorough assessment,early diagnosis,vigilant monitoring and agressive supportive managment in the intensive care setting.Therefore the knowledge of clinical features and the management of OP poisoning will help to take proper measures to improve the existing modalities for the better managment of such patients.
Objectives
1.To analyse and compile the prevailance,mode and motive of poisoning,clinical presentations,clinical features,management and out come of patients with OP poisoning admitted at Liaquat University HOspital Hyderabad/jamshoro. 2.To evaluate the adequacy of existing facilities at the intensive care units and medical wards of Liaquat University Hospital Hyderabad/jamshoro for the patients admitted with OP poisoning.
Material and Methods
Hypothesis
1.To analyse and compile the prevailance,mode and motive of poisoning,clinical presentations,clinical features,management and out come of patients with OP poisoning admitted at Liaquat University HOspital Hyderabad/jamshoro. 2.To evaluate the adequacy of existing facilities at the intensive care units and medical wards of Liaquat University Hospital Hyderabad/jamshoro for the patients admitted with OP poisoning.
Study Design : Descriptive Study
Sample Size : 50 patients
Sampling Method : purposive sampling
Duration of Study : six months
Inclusion Criteria
1.History taken from relatives about ingestion of OP poison.2.Clinical signs like exessive salivation miosis,bradycardia,wheezing,typical breath odour of OPs.`3.Respondin to atropine.
Exclusion Criteria
1:Patients with absence of miosis. 2:patints who expire in emergency department before reaching to intensive care unit or medical wards. 3:Age below 12 years.
Data Collection Procedure
data will be collected from 50 patients of OP poisoning admitted atICU and medical wards of LUH,Hyd/Jam.The proforna will be filled by asking questions from patients and attendants with details of personal habits.family and socioeconomic background and work environment.Complete psychiatric history will be obtained.Data about mode of presentation,clinical findings,management and complications will also be taken.
